Convert files gdb.base/[h-m]*.exp to use standard_output_file et al.

	* hook-stop-continue.exp, hook-stop-frame.exp, huge.exp,

	info-macros.exp, info-os.exp, info-proc.exp, info-target.exp,

28e7fd6234 Update years in copyright notice for the GDB files. 
Two modifications:
  1. The addition of 2013 to the copyright year range for every file;
  2. The use of a single year range, instead of potentially multiple
     year ranges, as approved by the FSF.

	Initialize mi->uiout depending on the mi_version as extracted from
	the interpreter's name.
	(mi_ui_out): New.
	(_initialize_mi_interp): Install mi_ui_out.  Adjust calls to
	interp_new.  Don't allocate the ui_out's of the interpreters here.
