	(md_show_usage): Add -xnone, -xdebugn, and -xdebugx. Relocate default
	indicator.
	(ia64_init): Set md.detect_dv.
	(ia64_start_line): New static variable warned. Warn only once when
	encountering explicit stops in automatic mode.
	* doc/c-ia64.texi: Describe -xnone, -xdebugn, and -xdebugx.
	* NEWS: Mention new default mode.
