* defs.h (xmalloc, xrealloc): Delete, they're declared in libiberty.h.
	(GETENV_PROVIDED, FCLOSE_PROVIDED): New.
	* doc/gdbint.texinfo (GETENV_PROVIDED, FCLOSE_PROVIDED): Document.
	* remote-sim.[ch] (gdbsim_write_stdout): New.
