(md_apply_fix): Likewise. Unify handling of BFD_RELOC_RVA,
	BFD_RELOC_32 and BFD_RELOC_16.
	(s_align, s_cpload, s_cplocal, s_cprestore, s_mips_stab,
	md_convert_frag, md_obj_end): Fix comment formatting.
